Who is a Thoracic Surgeon?
Thoracic surgeon are the ones that operate heart, lungs or food pipe also known as esophagus. Like every other surgeon they too take care of all the procedure related to the surgery. 1. Coronary Artery Bypass Surgery :Â
    Who needs this surgery?
      It is done to the patients whose coronary artery (a major artery to the heart) is blocked by fatty
     deposits also known as plaques. This surgery ensures that there is healthy blood flow and adequate
     oxygen is reaching the heart.
     Just like all the organs heart too needs constant supply blood.
     To do this, heart has two coronary artery on either side.
     With age and time these coronary arteries gets narrowed or gets blocked by plaques.
     This is called coronary heart disease and the process of blockage or narrowing of artery is called
     atherosclerosis.
     Causes of coronary heart disease
- age
- smoking
- overweight
- unhealthy diet with high fat in it
- lack of exercise or physical activity
- excessive drinking of alcohol
- due to other conditions like diabetes, high blood pressure, and high cholesterol
- family history of cardiovascular disease
     Tackling things like healthy diet and indulging in regular exercise could help in minimising the risk of
     coronary heart disease.
     What happens in the surgery?
     The surgery involves taking a blood vessel called graft from a different part of body and is attached
     above and below the blocked artery. The number of graft totally depend on the conditions of artery.
     The surgery is about 3 to 6 hours long and under the influence of general anesthesia.Â
2. Heart transplant
     A transplant is a surgery where a dysfunctional organ , which is heart in this case is replaced by
     healthier heart from a donor. Such surgeries are last resort after every other procedure or medication
     hasn’t been effective.
    When this surgery is performed?
      When all other treatments aren’t effective and the heart is deteriorating day by day leading to failure,
     a transplant is the only option left. Also when
- an individual has coronary artery disease
- somebody’s heart muscle is weak
- someone is suffering from heart disease
- or in some cases if the previous heart transplant has failed.
    Who might not be a good candidate for heart transplant?
- One who is older as the age may hinder the ability to recover from surgery.
- if you have another serious illness
- someone who has a history of cancer
- One who could not quit smoking or drinking as these are some habits that needs to be relinquished in order to keep the donor heart healthy.

     How to prepare for surgery?
     Preparation starts months before the transplant, the first thing you have to do is to seek evaluation
     from a heart transplant center as they will examine and final verdict will be made.
     As a patient you must look out for transplant center history like the number of transplant conducted
     at the center in year and how many of them were successful.
     The evaluation is carried to find that
- heart condition is capable to sustain transplant
- the patient could handle the treatment involved during surgery and post-surgery.
- the patient is willing to quit smoking or drinking.
- is the patient emotionally, physically and mentally strong and also has supportive network of close family member and friends to back him during the surgery.
     Risks are also involved in hear transplant like the failure of the heart or the body rejecting the donor
     heart or the arteries may develop problems like blocking or thickening restricting the blood flow. Post
     surgery medication involves intake of immunosuppressants which in turn increases the risk of
     infection, other diseases and cancer.
     Also there is the wait period before you get a donor’s heart. The demand for heart is more than the
     donor present. Hence the waiting period is long. Getting a donor heart also depends on your
     condition, blood group and size.
     If the waiting period is long and your condition is getting worse than the doctor might advice you to
     implant a ventricular assist devices that facilitates heart in pumping blood like shown in the image.
     The donor’s heart has life of four hours within which the transplant must take place. The surgery will
     be open heart procedure under the influence of general anesthesia. Post surgery that patient will
     stay in Intensive Care Unit for few days.Â
     Your condition will be closely monitor and upon successful transplant you can enjoy a healthy life.

3. Lung transplant
     A lung transplant is a surgical procedure which involves transplanting your ailing lungs with a new
     lung from a deceased donor. It is a procedure recommended by doctor when nothing seems to be
     working in improving the condition of your present lungs.
     Depending upon your case, single or both of your lungs could be replaced.

     Dysfunctional lungs puts survival of an individual in danger. There are different causes and condition

Thoracotomy procedures
     Thoracotomy is generally used when your lung has developed a cancerous tumor.

- Segmentectomy
     In this procedure a small segment containing tumor.
     In this surgical procedure the entire lobe is removed which contains the tumor including the lymph

- Extrapleural pneumonectomy
     In this the lung along with lining of the lungs, part of diaphragm and heart is removed.

     In this surgery the doctor inserts a instrument called thorascoscope which is a long tube like
     structure and has a camera attached to its head through a small incision. The doctor gets a high
     resolution image on his monitor and operate accordingly.
     It is almost similar to the above mentioned procedure because in this too the surgeon gets a high resolution video on his monitor via a camera inserted in the lungs of the patient. The surgeon operates on the console which then gets translated into instruction for the instrument inserted in the chest.
