What is Laparoscopic surgery?
Laparoscopy is a surgical procedure with minimal incision to examine the organs of our digestive system.
The surgery is done using a long thin tube like instrument called laparoscope which has a high definition camera and light at the tip. It is inserted into the body via small incision and the camera sends a real time image at the monitor.
It is low-risk, minimum invasive technique that allows patient to recover quickly as compared to the open surgery.
Benefits
The beauty of laparoscopic surgery is that a patient can go home within 24 hours after the surgery. However, there are some surgeries that may take longer than a day but marginally less than an open surgery. Let us have a look at some of the benefits of this surgery.
Small scars generally 1/2 inch long.
Saves lot of time and shorter hospital stay.
Less pain, fast recovery, since the incision is small risk of infection is also minimised.
Less damage to other internal organs.
Allows the doctor to perform two procedure at the same time. for example removal of appendix and ovarian cyst can be performed simultaneously.
Risks and Complications
Most common risks associated with laparoscopy are internal bleeding, infection and damage to the blood vessels or organs like stomach, bladder or ureters.
After the completion of surgery, it is sensible to observed your body closely and in case of any discomfort immediately contact your surgeon.
Look for these signs before contacting your surgeon
Chills and fever
Pain in the surgical area with the increasing intensity.
Any deformity near the surgical area like change in color, shape or size or bleeding.
feeling of discomfort or vomiting
difficulty in breathing
dizziness or vertigo
There are some other risk that could arise due to uncontrollable factors, in some cases immediate surgery helps. This includes
complications due to body reacting negatively to the anesthesia
discomfort arising from damaged organ while surgery
blood clots
When laparoscopy is used?
In treating and diagnosing number of conditions arising mainly in the abdomen, laparoscopy surgery is used. Hence, gastrointestinal surgeons and urologist use this surgery more often.
It is helpful in diagnosis of cancers like liver, cancer in organs located in the abdomen like gall bladder, pancreas, stomach, intestines etc. It is also useful in removing organs like appendix, gall bladder, womb, ovaries etc. Visit the following link if you want to more about when laparoscopy is used.
How it is performed?
It is performed under the influence of general anesthesia. Hence, you’ll have no recollection of the surgery. You can often recover from it on the same day itself.
Before the process
Depending upon the type of surgery you’ll be instructed to avoid any intake of food or water before few hours of the surgery.
You’ll be asked about your medication history and will be instructed to stop its consumption for few days before the surgery. Because some medicines could cause complication during the surgery.
In case of smokers, the patient will be advised to stop smoking before the surgery as it could delay the healing process after the surgery. It could also lead to complication or infection.
Most of the patients are discharged on the same day or the next day of the surgery. It is advised to arrange a driver to drive you back home as driving will stay out of bounds for the next 24 hours after the surgery.
The Surgery
A small incision is made in the abdomen by the surgeon. Incisions are 1 to 1.5 cm in size.
A tube called laparoscope is inserted through the incision. Then the abdomen is inflated with the help of some gases. This is done to provide a wide and clear view of the organs inside our abdomen. The view is rendered out on a screen by the laparoscope for the surgeon.
In case of removal of appendix or other small organs, other smaller in size surgical instruments are inserted to carry out the removal.
After the completion of the surgery the gases are extracted out of the abdomen followed by stitches and grouping.
Recovery
After the surgery, you may feel disoriented, sick or vomit due to the effects from anesthesia. These symptoms are common in various people and they pass very quickly.
You’ll be kept under observation by the nurse until all your vitals are normal.
Before the discharge you’ll be instructed about things, like how to keep your stitch clean, when to visit surgeon again or list of activities to be refrained during the recovery period.
In case of discomfort or pain due to the surgical wound you are advised to take painkillers to get some relief.
Cramps and body pain are some common conditions that arises due to the minimum amount of gases present in the abdomen.
With each passing day the body will react to the wound and use lot of energy for healing purpose. Hence a protein rich diet and complete sleep will help.
The time for recovery differs on individual, the condition of the patient for which the surgery was performed, overall health and many other vitals.
But 5 days are more than enough for you to recover and get back to your normal day to day activities.
Depending upon the reason for surgery the recovery time fluctuates. Like for removal of appendix 2-3 weeks are enough for recovery. But for removal of kidney or ovaries due to cancerous cell then the it gets extended up to as long as 11 weeks.
When to seek medical assistance after the surgery
To be on the safe side it advised to be under observation of someone after the surgery. If you experience symptoms like
High fever
experience chills
severe vomiting
high intensity pain
changes like swelling, bleeding near the surgical site
abnormal discharge or bleeding from vagina
swelling of arms or legs
You must contact the hospital where the surgery was performed or any other emergency service available.
Robotic Laparoscopy surgery
With the help of the advanced in the medical equipment robots have entered the field.
During a robotic laparoscopic surgery the surgeon has a console under his control. The console controls the robotic surgical arms which does the surgery on console’s command which is controlled by the surgeon.
The robotics arms too have a more powerful camera and surgical equipment. The powerful camera provides a more magnified 3D vision and a larger range for the surgical arm inside the body.
The arms also brings precision and helps the surgeon in carrying out complex surgical procedures. Gradually the use of robotic surgery is increasing in the countries like America, United Kingdom and India.
The precision also makes sure lower chance of complications than regular laparoscopy or open surgery.

Other defects that are rectified by this surgery are shell ears where the folds in the ear are absent, building of new ear for those who lost in injury or by-birth, ear lobe correction, increasing the size of ear. Ear pinning is more common between children up to age 15, followed by adults.
Short Term Side Effects of Surgery
Short term thumping or pulsating sensation in the ear, also pain. The surgery usually lasts for several long hours.
Hair fall and hair loss are terrifying for many people. It does not causes any discomfort to the patient but the level of psychological damage is collosal. In the last decade, the popularity of this treatment has significantly increased among the general population. Heredity, lifestyle or hormonal changes can be attributed as the cause of hair fall in men and women.
However, there are plethora of hair replacement techniques, although, these techniques are ineffective for person suffering from total baldness. Grafting and flapping are two most common technique that is used in which the hair is taken from the site where hair is still healthy. Scalp reduction and tissue expansion are some other techniques used by the surgeons. It may take several months (almost 2 years) to get the natural looking hair.
Short Term Side Effects of Surgery

Nasal surgery is performed to eliminate the obstructing in airflow or any other discomfort due to the nasal structure. Nasal septum, enlarged turbinates, narrowing of nasal opening are some of the conditions that are treated by these surgeries. Usually the doctors examine the nose and try to eliminate the obstruction through sprays, injection or oral medication. Generally, allergies causes the obstruction in passage of air via nose, hence doctors try to resolve allergies first, if the problem persists then surgery comes in picture. Surgeries include Septoplasty where the nasal septum is surgically straightened to open up the nostrils, Rhinoplasty where the structure of the nose is changed to eliminate the abnormalities in the structure and turbinate reduction to enhance the inflow of air.

It occurs when the fetus fails to join the upper lips together leaving an opening sometimes extending up to the nose trills during its developing stage inside the womb. It is common among boys. Also, point to be noted that people with cleft lip are otherwise healthy and no other irregularity or disability persists.
It may be stressful for the parents to adjust to the very noticeable cleft lip. There are different kind of cleft lip as shown in pictures above.
Causes of this condition are unclear. Some part of causes are attributed to the genes inherited from parents and the rest is still not completely understood by the scientists. Due to the involvement of genes, the chances of recurrence of cleft lip increases. Hence genetic consultation from the doctor is
Short Term Side Effects of Surgery
Short term difficulty in taking food, depending upon the age of the patient you can give them liquid diet. Breast milk is allowed for the infant. Care must be taken to protect the stitches after the surgery.
In some cases the skull of the baby tends to take up an odd shape while growing up. This can put pressure on the brain and such conditions are known as craniostenosis. Initially the brain is covered by a thin fibrous material called sutures.
Sutures allows the brain to grow in size and then hardens at a later stage sticking the skull bones together. In most of the cases the brain develops properly but the shape of the skull gets distorted. There are very rare cases where brain gets subjected to pressure due to shape irregularity restricting growth of the brain. Abnormal shape of skull at the time of birth or some weeks later is the only early symptom. The causes of this condition are not known but it is attributed to genetics. If you observe abnormal shape of your baby’s skull then it is not always a case of craniosynostosis. Suggest your doctor or surgery second opinion to get a clear picture.

It is inserted via small incision, the images captured by the camera are projected to a screen and with the help of it the surgeon maneuvers the endoscope. Actual surgery is performed by other equipment which is inserted from different incision.

Depending upon the type of flap and the component of the body that needs to be repaired there are different methods of flap surgery.
Local flap is a method where the surgeon takes the tissue from the nearest area (also called donor site) to the wound to cover the wound.
Regional Flap is the same method as the local flap the only difference is that the donor site is little further from the wound.
Bone flap in this the surgeon uses special tools to remove part of the bone. After the surgery the bone flap is restored back with the help of titanium screws and plates.
Musculocutaneous flap surgery, surgeon replaces the surgical site with a large chunk of muscle or tissue from the donor site.
Microvascular free flap uses microsurgery to cut the blood vessels and then join it back together.
Laser surgery
One of the most in demand kind of surgery, as the name suggest uses Laser as a medium to operate. There are many kind of lasers used depending upon the area and the purpose of the surgery. The advantages of laser surgeries are no scar, no bleeding and time efficient.

There are many different types of breast cancer. They are determined by the particular cells that are affected in different parts of the breasts. The doctors needs to determine two things before any specific diagnosis of the disease that is –
whether the disease has spread beyond breasts or not.
The tissue where it all began first.
Broadly breast cancers are divided into two, namely:
Non Invasive
Invasive
Non-invasive breast cancer
Non-invasive breast cancers occurs in milk duct or lobules (milk-producing glands) in the breast. They are contained at one place and do not spread out other parts. They are also known as carcinoma in situ.
Ductal carcinoma in situ (DCIS)
DCIS is a cancer that starts in milk duct and does not grow into rest of the breast tissue. Hence, it is not deadly but has a risk of developing into a invasive cancer at later stage.
Lobular carcinoma in situ (LCIS)
LCIS is a cancer that grows in lobules (milk-producing ducts) and does not spread to other breast tissues. Hence, it is not deadly but possess danger of developing invasive cancer in future.
This type of cancer spreads outside the duct or lobules of the breast affecting surrounding tissues.
Invasive ductal carcinoma (IDC)
IDC is the most common type of breast cancer constituting almost 80% of all the cases. The meaning is simple that the cancer which started from the milk ducts has spread out and affected the surrounding breast tissue. It has the potential to spread to lymph nodes and later to the other parts of the body. Therefore, early detection is of prime importance.
Invasive lobular carcinoma (ILC)
It is the second most common breast cancer after IDC. ILC means that the cancer which originated from the lobules of the breast spread past surrounding breast tissues. Over time it can spread to lymph nodes and to other parts of the body.
It is a rare type of breast cancer and it accounts for only 1% of all the recorded cases. It starts affecting the nipple, you will notice some changes on the skin of nipple or areola (the dark circle around the nipple) . Changes include skin being scaly, red, itchy and irritated. The cells which initiates these changes are known as Paget’s cell. Individual having this disease may also have either DCIS or LCIS breast cancer.
Inflammatory Breast Cancer (IBC)
IBC starts with swelling and reddening of breasts, no lumps here. It is a rare type of cancer, almost 1% of cases all over the globe. IBC spreads rapidly with symptoms worsening within days or even hours. Prompt treatment and the right advance diagnosis, however, is key in controlling the disease. Like the other kinds of breast cancer IBC also affects men.
Phyllodes tumors of the breast
Phyllodes means “leaf-like” and is a rare disease (less than 1% affected). The culprit cancerous cell grows in a leaf-like pattern. These cells are known as Phylloides. They rarely spread out of the breast but tend to grow at a fast pace. The phyllodes cells are found as benign, malignant and on border-line. Surgery is used to terminate the growth of the cells. The cells can occur at any age and a very rare in case of men.
Metastatic breast cancer
It is known as the stage IV cancer which means that the cancerous cells have escaped the breasts and have affected other organs like liver, lungs or brain. Getting diagnosed with this disease may send shivers down the spine but fortunately advanced medical technology are here to save the deal. Chemotherapy, surgery, radiation, hormonal and targeted therapy are some examples of the diagnosis that are advised by the oncologists. To know more about metastatic breast cancer click here.

Second opinion for breast cancer surgery in India is a very new concept. Second Opinion is in itself a very new concept for India. Although, people here indulge in seeking second opinion in every aspect of their life, especially when somebody from the family falls sick.
India has the largest number of medical colleges in the world, which in turn produces highest number of doctors in a year (approx. 50,000). But the population is increasing at a rate of 0.93% i.e. 1.3 crore a year creating a scarcity of doctors. This has also led to illicit practice of pretending to be a doctor in the country due to the high demand for doctors. Country treats doctors as God and this makes the job easy of such fraudsters.
These practices leads to number of mistreatment, false diagnosis and consumption of harmful medicines by the patients. This is where Surgery Second Opinion comes into play. They created a pool of experienced and skillful doctors working in apex medical institutions in India to provide second opinion related to surgery. It is has made the process online where one can upload his/her medical query and the respective doctor will state his/her opinion within 24 hours.

Second Opinion in Liver Transplant Surgery IS OF PARAMOUNT IMPORTANCE. It can be a difference between end stage liver failure and a second life. In todays scenario, live donor liver transplant is gaining popularity.

Liver transplantation surgically replaces a failing or diseased liver with one that is normal and healthy. At this time, transplantation is the only cure for liver insufficiency or liver failure because no device or machine reliably performs all of the functions of the liver. Some diseases in children also warrants a liver transplant.People who require liver transplants typically have one of the following conditions.
ACUTE LIVER FAILURE

Acute liver failure, also known as fulminant hepatic failure, occurs when a previously healthy liver suffers massive injury resulting in clinical signs and symptoms of liver insufficiency. Any number of things can lead to acute liver failure but the most common causes are acetaminophen overdose, viral infections, ingestion of a toxin such as poisonous mushrooms, or an idiosyncratic drug reaction.
The hallmark of this condition is the development of confusion (encephalopathy) within eight weeks after the onset of yellowing of the skin (jaundice). Confusion occurs because toxins typically metabolized by the liver accumulate. Unlike patients with chronic liver disease, who can survive weeks to months to years while awaiting liver transplantation, patients with acute liver failure may die within days if not transplanted.
CHRONIC LIVER FAILURE

The liver has a remarkable ability to repair itself in response to injury. Nevertheless, repeated injury and repair, typically over many years and even decades, scars the liver permanently. The end stage of scarring is termed cirrhosis and corresponds to the point where the liver can no longer repair itself. Once a person has cirrhosis, he or she may begin to show signs of inadequate liver function. This is termed “decompensated liver disease.” Although medications can decrease the symptoms caused by the liver failure, liver transplantation represents the only permanent cure.
Viral Hepatitis
Alcoholic Liver Disease
Metabolic Liver Disease
Autoimmune Liver Disease
Genetic Liver Disease
Vascular Liver Disease
Hepatocellular Carcinoma
What happens during a liver transplant?
Liver transplant surgery requires a hospital stay. Procedures may vary depending on your condition and your provider’s practices.
Generally, a liver transplant follows this process:
You will be asked to remove your clothing and given a gown to wear.
An IV (intravenous) line will be started in your arm or hand. Other tubes (catheters) will be put in your neck and wrist. Or they may be put under your collarbone or in the area between your belly and your thigh (the groin). These are used to check your heart and blood pressure, and to get blood samples.
You will be placed on your back on the operating table.
If there is too much hair at the surgical site, it may be clipped off.
A catheter will be put into your bladder to drain urine.
After you are sedated, the anesthesiologist will insert a tube into your lungs. This is so that your breathing can be helped with a machine (a ventilator). The anesthesiologist will keep checking your heart rate, blood pressure, breathing, and blood oxygen level during the surgery.
The skin over the surgical site will be cleaned with a sterile (antiseptic) solution.
The doctor will make a cut (incision) just under the ribs on both sides of your belly. The incision will extend straight up for a short distance over the breast bone.
The doctor will carefully separate the diseased liver from the nearby organs and structures.
The attached arteries and veins will be clamped to stop blood flow into the diseased liver.
Different surgery methods may be used to remove the diseased liver and implant the donor liver. The method used will depend on your specific case.
The diseased liver will be removed after it has been cut off from the blood vessels.
Your surgeon will check the donor liver before implanting it in your body.
The donor liver will be attached to your blood vessels. Blood flow to your new liver will be started. The surgeon will check for any bleeding where you have stitches.
The new liver will be attached to your bile ducts.
The incision will be closed with stitches or surgical staples.
A drain may be placed in the incision site to reduce swelling.
A sterile bandage or dressing will be applied.
The transplant evaluation process
If your provider thinks you may be a good candidate for a liver transplant, he or she will refer you to a transplant center for evaluation. Transplant centers are located in certain hospitals throughout the U.S.
You will have a variety of tests done by the transplant center team. They will decide whether to place your name on a national transplant waiting list. The transplant center team will include:
A transplant surgeon
A transplant provider specializing in treating the liver (a hepatologist)
Transplant nurses
A social worker
A psychiatrist or psychologist
Other team members such as a dietitian, or an anesthesiologist
The transplant evaluation process includes:
Psychological and social evaluation . Many different issues are assessed. They include stress, financial concerns, and whether you will have support from family or friends after your surgery.
Blood tests . These tests are done to help find a good donor match and assess your priority on the waiting list. They can also help improve the chances that your body won’t reject the donor liver.
Diagnostic tests . Tests may be done to check your liver and your general health. These tests may include X-rays, ultrasounds, a liver biopsy, heart and lung tests, colonoscopy, and dental exams. Women may also have a Pap test, gynecology exam, and a mammogram.
The transplant center team will review all of your information. Each transplant center has rules about who can have a liver transplant.
You may not be able to have a transplant if you:
Have a current or chronic infection that can’t be treated
Have metastatic cancer. This is cancer that has spread from its main location to 1 or more other parts of the body.
Have severe heart problems or other health problems
Have a serious condition besides liver disease that would not get better after a transplant
